Please call for fees. Payment is expected at the time of service. Zelle, check, and cash are accepted. Credit cards are not accepted. A full medical invoice with all billing and procedure codes necessary for insurance will then be generated and forwarded to you.
Insurance
Dr. Della Bella is out-of-network for all insurances and does not accept insurance assignment. He is not a medicare provider. Check with your insurance plan to see if it provides out-of-network benefits for psychiatric services and whether a referral or pre-authorization from your primary care physician is required for your visit.

Montclair, NJ 07042 The Crane Building, 460 Bloomfield Avenue, is on the Southeast corner of Bloomfield and South Fullerton Avenue. It is less than 10 minutes from Garden State Parkway Exit 148, Bloomfield Ave coming from the south, and Exit 149 Belleville Ave coming from the north (use your GPS from there). There is metered parking on Bloomfield Avenue and in The Crescent parking garage 1/2 block from the office, off South Fullerton Avenue. It is a vibrant area right across from the Church Street area of downtown Montclair and there are dozens of restaurants and coffee shops within a short walking distance. Accessibility: the office is on the second floor, reachable by stairs or elevator in the front lobby, and is fully wheelchair accessible. |
